LIABILITY RELEASE I, the undersigned, state that I freely and voluntarily am requesting the prayers and spiritual guidance of the Loudmouth Prayer Church and the Tulsa Healing Revival. I state that I clearly understand that the Staff of Loudmouth Prayer Church and the Tulsa Healing Revival are not professional Psychologists, Psychiatrists, Counselors, Medical Doctors, or Medical Experts but are Spirit-filled believers, trained in God’s Word. With this understanding, it is my desire to request spiritual assistance from the Loudmouth Prayer Church and the Tulsa Healing Revival, and do waive forever all liability and responsibility on the part of the Loudmouth Prayer Church and the Tulsa Healing Revival, and all functions and organizations connected thereto (either directly or indirectly), including but not limited to Directors, Officials, Officers, Staff, Employees, and all Personnel (both paid and volunteer), regarding any and all aid, assistance, direction, or guidance given or rendered to me at my request by the Loudmouth Prayer Church and the Tulsa Healing Revival Personnel as listed. By attending the Tulsa Healing Revival, I understand that I am entering an area where photography, audio and video recording may occur. My entry and presence constitutes my consent to be photographed and filmed which may be used on websites, in social media and marketing.
